Safety Engineering - TMA 2 Question 1
(a) A gas scrubber on an oilrig discharged gas periodically suchthat an accumulation of hydrocarbon gas nearby had occurred. Acontractor welder was dispatched to a job immediately adjacent tothe area and it being a shift change and personnel unavailabletemporarily, the welder decided to commence welding. As aconsequence, an incident occurred. Describe briefly:
(i) what you imagine was the incident including any hazardsexisting prior to or during the incident
(ii) the consequences of the incident on personnel, equipment orprocess
(iii) emergency procedures put into practice during andimmediately after the incident
(iv) indications of possible cause(s) of the incident (includeany breaches of safety procedures (permits to work, etc.) statutorysystems, codes of practice and company safety rules that led to theincident and any other causes or contributing circumstances
(v) recommendations/suggestions that would reduce thepossibility of a repetition of the incident.
